QIDRO Forms

These forms are designed to assist members of the State Employees’ Retirement System of Illinois (SERS) whose retirement benefits are potentially subject to division and distribution under a Qualified Illinois Domestic Relations Order (or QILDRO) during a marital dissolution proceeding.

QILDRO Consent form, QILDRO Order, QILDRO Calculation Order, and QILDRO Notice of Confidential Information. The forms may be completed online, printed for signatures, and submitted to SERS for processing. When you click in the first fill-in field on a form and complete that field, you should then hit the tab key to advance to the next field on the form.
